{"id":281,"date":"2016-07-12T17:42:47","date_gmt":"2016-07-12T17:42:47","guid":{"rendered":"https:\/\/www.modiohealth.com\/life\/?p=281"},"modified":"2017-03-06T21:21:36","modified_gmt":"2017-03-06T21:21:36","slug":"medical-credentialing-cost","status":"publish","type":"post","link":"https:\/\/www.modiohealth.com\/life\/credentialing\/medical-credentialing-cost","title":{"rendered":"Uncovering the True Cost of Medical Credentialing"},"content":{"rendered":"<p>This is the life of a typical healthcare practice manager:<\/p>\n<blockquote>\n<p style=\"text-align: left;\"><i><span style=\"font-weight: 400;\">It&#8217;s 9 PM on a Friday. You&#8217;re still at the office, scrambling to finish the initial credentialing applications for the new providers joining your practice. Your desk is covered with 2-inch-thick personnel files, but you&#8217;re still missing documentation for all three of the providers. The last time you spoke to Dr Smith, he was rifling through his attic for his MD diploma. Dr Miller can&#8217;t remember which CME course she took six months ago. And Dr White, who isn&#8217;t responding to your texts, calls, or emails, hasn&#8217;t finished his Delineation of Privileges form. Time&#8217;s running out, you&#8217;re exhausted, and in the back of your head, you&#8217;re wondering why your practice has to do all of this on paper. You&#8217;ve heard about credentialing software before, but it must be pretty expensive. Besides, there&#8217;s no way it could do everything you need &#8211; or else, why would you still be at the office?<\/span><\/i><\/p>\n<\/blockquote>\n<p>Sound like a nightmare? For many practice managers, this is real.<\/p>\n<p><span style=\"font-weight: 400;\">Here&#8217;s the problem: <\/span><strong><i>American healthcare is in crisis.<\/i><\/strong><span style=\"font-weight: 400;\"> The recent Obamacare health insurance law reform has brought in 32 million new patients (with an estimated <a href=\"https:\/\/www.aamc.org\/download\/150714\/data\">36 million <\/a>more joining Medicare in the next 20 years), but barely <a href=\"http:\/\/kff.org\/other\/state-indicator\/total-active-physicians\/\">100,000 physicians<\/a> currently practice in the U.S.<\/span><span style=\"font-weight: 400;\">\u00a0We desperately need more doctors, and yet physician credentialing &#8211; the very process that vets a doctor for practice &#8211; remains the most broken, outdated system in the business.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Cost is the first (and the worst) issue. Every year, your medical practice could spend over <a href=\"http:\/\/www.mgma.com\/about\/about-mgma-medical-group-management\/about-center-for-rese\">$7,000<\/a> on each provider&#8217;s credentialing application.<\/span><span style=\"font-weight: 400;\">\u00a0The average physician spends 43 minutes a day interacting with health plans instead of their patients while administrators spend 20 hours to credential every provider. Administrative tasks are so demanding that practices lose nearly <a href=\"http:\/\/www.mgma.com\/about\/about-mgma-medical-group-management\/about-center-for-research\/analyzing-the-cost-of-administrative-complexity\">$100,000<\/a> in staff wages every year. (If you belong to a specialty group, that number goes up.)<\/span><span style=\"font-weight: 400;\">\u00a0Still, it&#8217;ll take 2-3 months before that new doctor can even start working&#8230; if everything goes smoothly. MCOs have strict application guidelines, and it&#8217;s tough to jump through all the hoops. One requirement is proof of malpractice insurance, which can range from <a href=\"http:\/\/work.chron.com\/much-doctors-pay-insurance-7304.html\">$10,000-$100,000<\/a> or more a year per provider.<\/span><span style=\"font-weight: 400;\">\u00a0You might get sticker shock, but insurance isn&#8217;t optional &#8211; not when medical errors are the <a href=\"http:\/\/www.npr.org\/sections\/health-shots\/2013\/09\/20\/224507654\/how-many-die-from-medical-mistakes-in-u-s-hospitals\">3rd leading cause of death in the US<\/a>.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Clerical medical errors can also cause giant problems. Up to <a href=\"http:\/\/www.physicianspractice.com\/payers\/five-steps-easy-physician-credentialing\">85%<\/a> of credentialing applications are incomplete.\u00a0<\/span><span style=\"font-weight: 400;\">Leaving dates blank or writing a &#8220;See CV&#8221; note saves time, but the application will still get rejected. Is the provider&#8217;s state license still pending? Good luck. The average 60-90 day wait for credentialing can quickly become twice that, especially since health plans won&#8217;t process an incomplete application. <\/span><em><span style=\"font-weight: 400;\">Can your practice wait that long?<\/span><\/em><\/p>\n<p><span style=\"font-weight: 400;\">If your physicians aren&#8217;t credentialed in time, it&#8217;ll put the brakes on patient scheduling, delay billing and coding, and expose patients to a doctor who hasn&#8217;t been fully vetted. Medicare and Medicaid both block un-credentialed providers from billing for their <a href=\"http:\/\/medicaleconomics.modernmedicine.com\/medical-economics\/news\/modernmedicine\/modern-medicine-now\/billing-noncredentialed-providers\">services<\/a>. Skirt this rule by billing under another credentialed doctor&#8217;s name, and you&#8217;re looking at an audit.<\/span><span style=\"font-weight: 400;\">\u00a0Plus, a provider without credentials hasn&#8217;t yet cleared the background checks, affiliation requests, and other security measures required for patient safety.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-283 alignright\" src=\"https:\/\/www.modiohealth.com\/life\/wp-content\/uploads\/Screen-Shot-2016-07-12-at-10.26.15-AM-1024x687.png\" alt=\"Screen Shot 2016-07-12 at 10.26.15 AM\" width=\"457\" height=\"307\" srcset=\"https:\/\/www.modiohealth.com\/life\/wp-content\/uploads\/Screen-Shot-2016-07-12-at-10.26.15-AM-1024x687.png 1024w, https:\/\/www.modiohealth.com\/life\/wp-content\/uploads\/Screen-Shot-2016-07-12-at-10.26.15-AM-300x201.png 300w, https:\/\/www.modiohealth.com\/life\/wp-content\/uploads\/Screen-Shot-2016-07-12-at-10.26.15-AM.png 1097w\" sizes=\"auto, (max-width: 457px) 100vw, 457px\" \/><\/p>\n<p><span style=\"font-weight: 400;\">What<\/span><i><span style=\"font-weight: 400;\"> can<\/span><\/i><span style=\"font-weight: 400;\"> you do, then?\u00a0<\/span><span style=\"font-weight: 400;\">Here&#8217;s some quick tips to ensure that your team of physicians is fully credentialed (and getting paid on time):<\/span><\/p>\n<ul>\n<li><b>Hire credentialing coordinators (or train your management staff). <\/b><span style=\"font-weight: 400;\">Spare your medical staff the administrative burden. Can&#8217;t afford another worker? Hold a training session for your staff, and join the <\/span><a href=\"http:\/\/namss.org\/\"><span style=\"font-weight: 400;\">NAMSS<\/span><\/a><span style=\"font-weight: 400;\"> or <\/span><a href=\"http:\/\/www.mgma.com\/\"><span style=\"font-weight: 400;\">MGMA<\/span><\/a><span style=\"font-weight: 400;\"> network so you&#8217;re equipped with the right resources. \u00a0<\/span><\/li>\n<li><b>Move your records to the cloud (or at least to your computer).<\/b><span style=\"font-weight: 400;\"> What happens when a new practice asks you for your previous doctor&#8217;s work history? You don&#8217;t want to ignore that request (it&#8217;s not worth the <\/span><a href=\"http:\/\/www.hg.org\/article.asp?id=6854\"><span style=\"font-weight: 400;\">liability risk<\/span><\/a><span style=\"font-weight: 400;\">), but you don&#8217;t want to dig through those 15-year-old file cabinets, either.<\/span><span style=\"font-weight: 400;\"> If you still need a good horror story to convince you, check out the <\/span><a href=\"http:\/\/www.duanemorris.com\/articles\/static\/modern_healthcare_aug06.pdf\"><span style=\"font-weight: 400;\">Kadlec case<\/span><\/a><span style=\"font-weight: 400;\">. Keeping records in the cloud could spare you a lawsuit. <\/span><\/li>\n<li><b>Use physician management software. <\/b><span style=\"font-weight: 400;\">There are plenty of software packages available for physician management, but they&#8217;re not all equal. A good system should be web based, ensuring easy access to the software for all staff. Expect your software to keep track of licenses, have unlimited documented uploads, electronic signatures, CAQH integration and include primary source verification for your providers&#8217; data. Run a cost-benefit analysis, too: often the biggest, flashiest names in the market aren&#8217;t offering the best deal. \u00a0\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">In the end, while you can&#8217;t dodge credentialing fees, you can avoid expensive mistakes.
